How we will support the people that you refer? If we think we can support the person that you have referred, we will agree to do an assessment of need. Once we completed this , we will tell you if we are the correct provider and will offer our service intention in letter form. This will detail the costs, the service we will offer and what you should expect from us. It will also detail service start timescales that include transition. This will also include risk management processes such as HCR20 and formulation of needs and support plans; to ensure our bespoke design fits with the individual. We will work in partnership to manage restrictions and DOLs.
